Who we are
Mobile Hearing Solutions is a mobile hearing-care provider serving the Dallas–Fort Worth (DFW) area and surrounding communities. We travel to clients’ locations and do not publish a physical address for safety and privacy. Contact: Ashley.bukowski@mobilehsolutions.com, (469)613-4327.

Scope
This policy explains how we collect, use, disclose, and protect personal information when you visit our website, communicate with us, use our online tools (including any online hearing screening), or purchase products/services. It applies to U.S. visitors and may also apply to visitors from other jurisdictions.

Information we collect

• Identifiers: name, email, phone, IP, device identifiers

• Contact & account details: preferences, messages

• Health information (PHI): hearing/medical info, screening responses, provider notes you submit for care

• Insurance information: member ID, DOB, plan details, information submitted to verify benefits

• Transaction data: orders, payment confirmations (card data handled by processors)

• Internet/device activity: pages viewed, timestamps, cookies/pixels

• Geolocation (approximate): from IP or user entry for service coverage

How we collect it

• Directly from you via forms, booking tools, online screenings/tests, email, text, and phone

• Automatically via cookies and similar tech when you use our site

• From third parties (insurers, payment processors, service providers acting on our behalf)

How we use information

• Provide, schedule, and manage mobile hearing-care services and telehealth

• Verify insurance eligibility/benefits; obtain authorizations; process claims (as permitted)

• Fulfill current and future online sales (hearing aids/accessories)

• Communicate confirmations, reminders, and support

• Improve services and website; maintain security and prevent fraud

• Comply with legal, regulatory, and audit requirements

How we share information

• Service providers/Business Associates under contracts and BAAs (forms, scheduling, CRM, secure messaging, cloud hosting, analytics)

• Insurance companies/payors to verify eligibility/benefits or process claims as authorized and permitted by law

• Payment processors to complete transactions (we do not store full card data)

• Professional advisors bound by confidentiality

• Law enforcement/regulators or as required by law or to protect rights/safety
  We do not sell your personal information for money and do not share PHI for targeted ads.

Cookies & analytics
We use essential and analytics cookies. You can adjust browser settings to reject cookies (site functionality may be affected). Where required, we will present consent tools.

Security
We use reasonable administrative, technical, and physical safeguards. No system is perfectly secure.

Retention
We retain PHI and business records consistent with healthcare requirements and applicable laws. When no longer needed, we securely delete or de-identify.

Children’s privacy
Services are for adults and for minors only with parent/guardian involvement and a qualified provider. We do not knowingly collect info from children under 13 without appropriate consent.

Your rights
Depending on your location, you may request access, correction, deletion, or a copy of personal information, and request restrictions/objections to certain processing. Do-Not-Track / Global Privacy Controls
Our site may not respond to DNT signals. We honor legally recognized Global Privacy Controls where required.
